ACAW Trust Funds Notice of Address Change

If your address has changed, please print and complete the Notice of Address Change Form. Completed forms can be submitted in person, by mail, fax, or e-mail. This form is for ACAW Trust Funds internal use only. Please contact your Local Union office to update your address with them.

Health & Wellness Dental Claims & Weekly Disability Emails

Dental claim forms can now be emailed to the ACAW Trust Funds office. Please do not use any SIN. You may use the member’s UBC or Unique ID number only. Dental claim forms must be signed – dental offices cannot sign on behalf of a patient, nor is “Signature on File” acceptable. Signed claim forms can be emailed to dentalclaims@acawtrustfunds.ca.

A copy of your Weekly Disability claim forms can now be emailed to the ACAW Trust Funds office. Please do not use any SIN. You may use the member’s UBC or Unique ID number only. Completed forms can be emailed to wdclaims@acawtrustfunds.ca to start the Weekly Disability claim process. Please note that the original Weekly Disability claim form package, including Service Canada CPP form 1618A must be received in office prior to any payments being issue. Section 2 of the Service Canada form is to be completed by ACAW Trust Funds staff.

Changes to the Calculation of Commuted Values

January 1, 2023

Dear Plan Members and Beneficiaries of the ACAW Pension Plan

Changes made in 2018 to the Regulations to the Alberta Employment Pension Plans Act has allowed defined benefit pension plans that are a Collectively Bargained Multi-Employer Plan (CBMEP) to elect to have commuted values* of pension benefits determined in accordance with the actuarial assumptions used in the current actuarial valuation report multiplied by the lesser of 1 and the going concern funded ratio. Health & Wellness

Direct billing by service providers has been limited to Dental, Drugs, Ambulance and Hospital Room Charges. ALL MEMBERS WILL BE REQUIRED TO FIRST MAKE PAYMENT TO ALL OTHER SERVICE PROVIDERS and then submit their claims to the ACAW Trust Funds office. There will be no exceptions to this notice.
